Starting the week of March 12th, 2012, I will be accepting appointments for registrations for the 2012-2013 school year. Please call 819-776-3158 ext. 2303 to book your appointment between 1:00 and 4:00 pm Friday March 23rd in Poltimore.
Required documents for registration:
- Child’s original long form birth certificate (bearing parents’ names)
- Certificate of eligibility of the child, sibling or parent
- Canadian Citizenship of parent
- Québec driver’s license of the parent/guardian responsible for the child
- Québec proof of residency (ex: lease/house deed AND tax bill or
- hydro/cable/phone bill)
- Child’s last report card (if applicable)
